New Parents: Please complete the form below to register for your intake orientation.

After you’ve registered, you can prepare for orientation by gathering:

• A copy of children's birth certificates

• Verification of employment or enrollment in school (ex. recent paystub or enrollment verification form)

• Verification of address (your name on a utility bill, rental agreement, etc.). If your name does not appear on any bills, you must bring an affidavit from the person whose name is on the bill stating that they live with you full-time. You can request a copy of the affidavit from us.

• Documentation for any food allergies, if applicable

• PRESCHOOL ONLY: Up-to-date immunization records or notarized immunization exemption

• PRESCHOOL ONLY: Health statement signed by the children's physician (please request a copy of this form if needed)

Releases and intake forms will be sent to you via email. They can be digitally signed ahead of time or signed at orientation.

At orientation, you will also be asked to complete income eligibility paperwork detailing the income of everyone in your household.
