Returning Parents: Please complete the form below to register for your annual orientation.

After you’ve registered, you can prepare for orientation by gathering:

  • A copy of children's birth certificates, if they are not already on file with us

  • Verification of employment or enrollment in school (ex. recent paystub or enrollment verification form)

  • Updated medical information for any food allergies, if applicable

  • PRESCHOOL ONLY: Updated immunization records

  • PRESCHOOL ONLY: Health statement signed by their physician, if it is not already on file with us (please request a copy of this form if needed)

Releases and intake forms will be sent to you via email. They can be digitally signed ahead of time or signed at orientation.

At orientation, you will also be asked to complete income eligibility paperwork detailing the income of everyone in your household.
